…received her education in San Jose California where she graduated from Palmer West Cum Laude in 2014. She earned her undergraduate degree in Biology at the University of Alberta. During her time in chiropractic school she received the Clinical Excellence Award signifying outstanding service to patients as nominated by faculty members. Dr. Fayad also travelled to India as part of a Clinic Abroad Program where she provided chiropractic care to under-served children. Dr. Fayad is passionate about her patients and works with them to provide individualized, evidence-based treatment plans to promote optimal health. From pediatrics to pregnancy to professional athletes--Dr. Fayad loves treating a variety of populations. She is currently certified in Active Release Technique, Webster Technique, Rocktape & Instrument Assisted myofascial release.
Dr. Fayad is a regular on the Global Morning Show providing tips to Canadians on injury prevention. She has been featured in many magazines including Canadian Chiropractor, Neighbours of Glenora and Avenue Edmonton. Find her daily serving health & wellness tips on her popular Instagram page @drmeccafayad where she packages chiropractic medicine in an approachable, relatable way.
During her free time Dr. Fayad enjoys reading, yoga, cycling, travelling & baking. She also loves visiting her hometown of Lac La Biche.

DR. NAOMI VELADO-TSEGAYE

…was born and raised in Edmonton, Alberta. She completed her undergraduate studies at the University of Alberta in Immunology and furthered her education in San Jose, California where she graduated from Chiropractic School at Palmer West with Honors.
Dr. Naomi aims to address the root cause of patients’ pain or injury and uses a combination of techniques such as cupping, taping, MRT, and IASTM to promote spinal wellness. She enjoys getting to know her patients and learning their life stories. She is passionate about her work and believes that human connection is an important part of the healing process.
In her spare time, you can find her painting, playing soccer, hiking, cycling, or enjoying time with friends and family.

PHYSIOTHERAPIST
… Vincent has practiced in Alberta, British Columbia, and Virginia over the past nine years. Vincent completed his Bachelor’s of Human Kinetics at the University of British Columbia (UBC) in 2010 and continued his studies there for his Master’s of Physiotherapy, completed in 2012. Since graduating, he has pivoted away from a purely structural approach, embracing the complexity and challenges faced when working within a more holistic person-centred model. He has competed and regularly participated in volleyball, soccer, and enjoys kayaking and going for walks with his whoodle, Charlie.
From a personal and professional outlook, Vincent understands that rehab, training, and performance all exist on a continuum. He recognizes that establishing skilled movements for performance and in daily life can help build confidence which in turn allow for long term sustainable change. He actively works with his clients, building resiliency and self-efficacy through a combination of manual therapy, exercise, and education. Vincent has a special interest in working with thoracic outlet syndrome, runners, and knee (patellofemoral) pain. While working with all clients, he is a movement optimist, believing in the variability and the durability of the body and the inherent ability to adapt and improve given the right stimulus.

…is a Registered Massage Therapist with CRMTA. He graduated from MaKami College in 2018 with 3000 hours of clinical experience and was awarded with a Certificate of Distinction in Advanced Clinical Massage Therapy.
Since then, Michael has helped clients with repetitive stress injuries, postural dysfunction, chronic pain, migraines and more. His multifaceted style ranges from Swedish relaxation to therapeutic deep tissue techniques. He offers Dynamic Cupping, Assisted Stretching and RAPID-NFR in his treatments to promote healthy movement in the joints and tissues.
Michael is motivated to help his community live pain free and to achieve a better quality of life through massage therapy. He strongly believes in the bodies natural ability to heal given the proper care, treatment plan and knowledge. During his free time, Michael enjoys getting outside exploring the river valley, practicing yoga, travelling, shopping the local farmers market and spending time with his friends and family.

Destiny is a Registered Massage Therapist who graduated from the West Coast college of Massage Therapy.
Her goal is to provide personalized care to assist each patient in reaching optimal health. To provide long term resolution, she looks for the root cause of your problem while addressing symptoms for immediate relief.
She uses a variety of techniques and tailors each treatment to your specific individual needs. Her current methodology contains a mix of Swedish techniques, Myofascial Release, Deep Tissue modalities, and Neuromuscular Facilitation techniques. She has a passion for injury rehabilitation and prevention and experience working with a broad range of patients, including athletes, pre- and post-natal, motor vehicle accidents, and those with overuse injuries or postural dysfunctions. Her approach uses soft tissue manipulations during treatment, combined with stretching and strengthening recommendations as home care, to achieve optimal therapeutic effect .
SANDRA STRANGMAN

… Sandra has been in practice for over 28 years as a registered massage therapist. Over the years, Sandra has worked with many groups including women’s hockey, Olympic weightlifting, ball hockey, women’s soccer and most notably, The Shumka Dancers.
She has a keen interest in athletics and enjoys augmenting her clients’ athletic performance. While Sandra has a wide and varied clientele, many of her clients are Masters Athletes who benefit from her wide range of massage techniques including kinesio taping. She specializes in deep tissue, sports massage, and acupressure.

… Chris is a registered massage therapist that graduated in 2021 with honours from Makami College in Edmonton, Alberta.
"Massage has so many benefits for physical, mental, emotional and spiritual wellbeing. Pain relief from sore muscles, increased flexibility, relaxation, mental clarity, improved circulation, sports recovery, and injury prevention. Chris wants to help others achieve optimal health and exceed their goals. He is passionate towards providing a sense of well-being to his clients and has spent hours on treating clients with acute, sub-acute and chronic conditions. He is skilled in Swedish, Sports, Trigger Point Therapy, Deep tissue, Kinetic techniques, nerve flossing, and myofascial release.
In his spare time Chris enjoys visits with family and friends, travelling, playing ice and in-line hockey, and pretty much any outdoor activity/adventure.

… Lesley has been in the healthcare field for over 8 years as an Office Manager. She brings extensive knowledge in customer experience, sales, human resources and detailed administrative processes. With experience in retail management, Lesley learned early on the importance of intrapersonal skills, developing client relationships and working with an inclusive team. She is skilled at working effectively to coordinate information and solve problems.
She is a passionate wood worker and enjoys building custom creations such as furniture and Christmas crafts (best known for her wooden snowmen at local Edmonton Christmas markets). As an elite level athlete in Ladies ‘AA’ Fastball and basketball from a young age, she has developed a strong passion and love for sports and a healthy lifestyle. Lesley enjoys spending time with her niece and nephews. In the Summer you can find Lesley teaching her nephew mad basketball skills, enjoying the trails in our beautiful city, or in her garden. As an avid gardener Lesley is a member of Community gardens and enjoys growing vegetables and over taking her patio with flowers.

LAUREN MCDOWELL

… Is currently in her second year of a bachelor of Kinesiology degree at the University of Alberta. She is hoping to pursue her lifelong dream of becoming a physiotherapist, by getting her MScPT (Master of Science in Physical Therapy). Due to injuries encountered from playing soccer and other high school sports she was recommended chiropractic care, and that led her to cross paths with Dr. Fayad.
Lauren has worked alongside Dr. Fayad for the last year behind the scenes, by helping grow her Instagram, create The Chiropolitan (Dr. Fayad’s Newsletter) and plan social events for her lovely clients. Lauren is excited to now be a part time receptionist at Form Health to further her knowledge in the health and wellness practice, as well as the business side of opening and owning a clinic. She cannot wait to welcome you to Form Health, get you a coffee and set you up on the Normatec boots!
When Lauren isn’t studying, she is found hanging out with friends, travelling with family, trying to stay active and eating pizza!

…was born and raised in Saskatchewan. After graduating she moved to Edmonton, Alberta to pursue a Bachelors of Psychology degree at the University of Alberta. She has always been pursuant of a career where she can help others and this is what drew her to Form Health.
Jala joined the clinic in August of 2022 as a receptionist. She’s here to help book your appointment, show you around the clinic, and answer any questions!
